We offer our Surrogate Screening Package, designed to thoroughly screen and prepare your surrogate, along with three levels of Case Management to provide ongoing support once your match is in place.
Our Surrogate Screening Package includes the following:
Intake with your Surrogate - We review medical and pregnancy history, discuss the surrogacy process, answer questions related to the surrogacy journey, and confirm compensation and journey expectations.
Medical Records - OB, delivery, and IVF records as well as most recent pap and physical will be requested for all previous pregnancies. All records will be reviewed by a nurse for complications and disqualifying factors based on your clinic’s requirements.
Background Check - This is completed for all adults over the age of 18 in the household. It consists of a nationwide, state, and county civil and criminal search, sex offender registry search, DMV check, social security number trace, social media search, and drug screen.
Virtual Home Visit – While on a video call with one of our social workers, the surrogate will do a walk through her home to help us ensure that she lives in a safe environment.
Psychological Evaluation - This consists of four parts: a personality test for your surrogate; an interview with your surrogate and her partner conducted by a licensed mental health provider; an IP session with the licensed mental health provider; and a group match meeting with the IPs, surrogate, surrogate’s partner, and licensed mental health provider.
Insurance Review – Policy verification is completed to determine if the insurance policy is surrogacy friendly and can be used for OB and Delivery care for your surrogate. This is a review-only service and does not include quotes, recommendations, or coordination of any insurance products.
OB, delivery, and IVF records as well as most recent pap and physical will be requested for all previous pregnancies. This service is included in our screening package but is available as a stand-alone service or if your initial surrogate’s records were not approved at your clinic and you need records collected for another potential surrogate.
We provide comprehensive coordination through the early stages of your journey, including appointment scheduling, travel arrangements, assistance with escrow reimbursements, and ongoing communication with all parties involved through the start of OB care.
To support both Intended Parents and Surrogates, we offer monthly support groups designed to ensure everyone feels prepared, protected, and supported throughout the process.
This package also includes a 3-month, $500,000 life insurance policy for your surrogate, beginning at the medical screening appointment. After this period, intended parents are responsible for maintaining any necessary insurance coverage.
This package includes full coordination and support from match through 12 weeks post-delivery. Services include appointment scheduling, travel arrangements, assistance with escrow reimbursements, and ongoing communication with all parties involved.
Both the Intended Parents and the Surrogate receive five sessions with a licensed social worker. Monthly support groups are also provided to foster a safe, smooth, and well-supported experience throughout the journey.
A 3-month, $500,000 life insurance policy for your surrogate is included, beginning at the medical screening appointment. After this period, intended parents are responsible for maintaining any necessary insurance coverage.
Our most comprehensive offering, this package provides full-service coordination from match through escrow closure (typically around 6 months post-delivery). This includes appointment scheduling, travel arrangements, escrow reimbursement support, and ongoing communication with all parties involved.
Both the Intended Parents and the Surrogate receive up to 12 sessions with a licensed social worker. Monthly support groups are also included to ensure continued guidance and emotional support throughout the journey.
This package includes a 12-month, $500,000 life insurance policy for your surrogate, beginning at the medical screening appointment. After this period, intended parents are responsible for maintaining any necessary insurance coverage.
Additionally, Intended Parents may choose between birth planning assistance or a birth and newborn course through Childbirth Allies.
